Programme overview
Evidence for Health (E4H) is a Foreign, Commonwealth & Development Office (FCDO)-funded programme aimed at strengthening Pakistan's healthcare system, thereby decreasing the burden of illness and saving lives. E4H provides technical assistance (TA) to the Federal, Khyber Pakhtunkhwa (KP), and Punjab governments, and is being implemented by Palladium along with Oxford Policy Management (OPM).
Through its flexible, embedded, and demand-driven model, E4H supports the government to achieve a resilient health system that is prepared for health emergencies, responsive to the latest evidence, and delivers equitable, quality, and efficient healthcare services. Specifically, E4H delivers TA across three outputs:
Output 1: Strengthened integrated health security, with a focus on preparing and responding to health emergencies, including pandemics.
Output 2: Strengthened evidence-based decision-making to drive health sector performance and accountability.
Output 3: Improved implementation of Universal Health Coverage, with a focus on ending preventable deaths.
Background
Khyber Pakhtunkhwa (KP) has made significant strides in reforming its health sector. The province operates under its own Khyber Pakhtunkhwa Health Policy , which prioritises strengthening primary and secondary healthcare, improving governance, enhancing the quality of services, and addressing the burden of communicable and non-communicable diseases.
- The KP health system continues to face challenges, including poor planning, inadequate financial resources, and insufficient essential services, as well as issues with accessibility and equity, especially in National Health Support Programme (NHSP) priority districts. Barriers also include geographic isolation, inadequate infrastructure, shortages of skilled workers, weak health information systems, and inefficiencies in financial management. These challenges lead to poor health outcomes, particularly among vulnerable populations.
- A pivotal initiative designed to support these goals is the NHSP. The NHSP aims to strengthen health systems across three provinces in Pakistan, with a core focus on accelerating progress towards Universal Health Coverage (UHC). The programme's effectiveness is driven by a results-based financing modality, utilizing Disbursement-Linked Indicators (DLIs). These DLIs focus on ensuring that PHC facilities meet essential service delivery norms, particularly in areas with lagging performance, while also incorporating climate risk considerations.
- The NHSP seeks to address these challenges by strengthening health systems, promoting equity, and improving health outcomes across KP. A key performance indicator for accessing NHSP budget support through the recurrent budget is the development of District Action Plans (DAPs), which should also translate provincial priorities, including the Essential Package of Health Services (EPHS), into context-specific, actionable reforms at the district level
Problem Statement
- There is a recognised gap in the capacity of health management teams in NHSP prioritised districts to develop robust, evidence-based, and costed action plans that align with NHSP objectives and provincial health priorities. Existing plans (if available) often lack a strategic focus or are not sufficiently monitored, resulting in implementation inefficiencies and suboptimal health outcomes. There is an urgent need to provide technical assistance to selected priority districts in KP to develop comprehensive, realistic, and measurable DAPs that will directly contribute to accessing NHSP budget support.
Goal and Objective(s)
The goal is to develop evidence-based, cost-effective, and implementable DAPs for NHSP priority districts in KP to access financing, thereby strengthening local health systems and improving service delivery.
Specifically, to:
- Translate provincial health priorities (KP Health Policy, EPHS, NHSP) into district-level interventions.
- Under the leadership of Health Department, KP, facilitate a collaborative planning process with District Health Offices (DHOs) and key stakeholders to define strategic priorities and interventions.
- Develop comprehensive DAPs for the prioritised four districts (in each division), with clear objectives, activities, timelines, responsibilities, and a detailed budget.
- Strengthen the capacity of district health teams in strategic planning, budgeting, and monitoring & evaluation.
- Establish a robust monitoring framework for each DAP to track progress and ensure accountability.
Deliverables
The Public Health Expert will work on the following Deliverables.
Lead desk reviews, conduct situational analyses, facilitate district-level planning workshops, support DHMTs in developing the draft the DAPs.

Background
Pakistan is committed to the 2030 Agenda for Sustainable Development. In health, Universal Health Coverage (UHC) is the key policy goal. The Human Resources for Health (HRH) Vision 2018–30 and the upcoming National Health and Population Policy (NHPP; both aim to address workforce shortages and enhance national and provincial capabilities for health workforce planning and development. The HRH vision emphasizes strategies, governance, legislation, and resources required to ensure a workforce of appropriate size, skills, and distribution to meet people's needs.
The Pakistan Nursing Council (PNC) Act 1973 is the cornerstone for regulating Pakistan's nursing profession. Amended by the National Assembly of Pakistan on Thursday, 19th January 2023. The amendment act is called the Pakistan Nursing and Midwifery Council (Amendment) Act, 2023.
The act empowers the PNMC to set educational standards, license and register practitioners, regulate professional practice, and ensure continuous professional development, quality assurance, and accreditation of academic institutions. To effectively implement the act, the Pakistan Nursing & Midwifery Policy Framework (PNMPF; was developed with the support of the Evidence for Health (E4H) Programme and under the directives of the Prime Minister of Pakistan. The PNMPF identified revamping the PNMC secretariat as one of its key priorities, along with reorganizing the management, education, and quality assessment units within the council's secretariat.
Problem Statement
The PNMC Secretariat currently faces significant structural and operational challenges that limit its ability to effectively regulate the nursing and midwifery professions. Key issues include an outdated organisational structure, limited human resource capacity, weak accreditation processes, and the absence of modern digital systems. These gaps result in inefficiencies in core functions, including institutional accreditation, professional licensure, quality assurance, and ongoing professional development. To address these challenges, there is an urgent need to review the Secretariat's functions as outlined in the Act, reorganize its structure and key units, and build stronger regulatory capacity. This will help improve performance, enhance service delivery, and ultimately strengthen the quality of the nursing and midwifery workforce across Pakistan.
Goal and Objective(s)
The goal is to revamp the PNMC Secretariat to strengthen the regulatory capacity of the council for a qualified, skilled, and ethical nursing and midwifery workforce that contributes to achieving UHC and improved health outcomes in Pakistan.
The objectives are to Conduct a rapid assessment of the existing PNMC Secretariat to identify gaps in the mandate, workflows, staffing, skills, and systems.
Develop a revised organisational structure (organogram) of the PNMC Secretariat, through a consultative process and under the guidance of the Joint Secretary (Admin), while considering provisions in the Act.
Develop clear job descriptions / TOR of all staff of the secretariat and three units: 1) Management, 2) Education, and 3) Quality Assessment, etc., with clear responsibilities, reporting lines, staffing norms, appointment criteria, competency profiles, and key performance indicators (KPIs) for an efficient and modernised secretariat.
Identify staff positions that could be outsourced, along with a detailed description of their associated processes.
Strategic Approach
Contributions to health systems strengthening: This activity will directly contribute to health systems strengthening by enhancing the regulatory oversight and governance of a critical segment of the health workforce. Additionally, improving the quality of education, training, and professional standards for nurses and midwives has a direct impact on service delivery in both the public and private sectors, as well as on the delivery of results.
Alignment with other E4H TAs/Investments/National Standards:The TA activity is aligned with the HRH Vision , the PNM Framework , and the upcoming draft National Health & Population Policy
Alignment with other donors (if relevant): No direct financing from different development partners is foreseen in this TA. However, key stakeholders, including WHO, will be consulted throughout the process to ensure ownership.
Scope of Work and Methodology
The team will work closely with the Ministry of NHSR&C and PNMC. Below is a list of the sequence of activities that the consultants will be expected to undertake.
A. Inception Phase
Review relevant legislation, policies, strategies, and the mandate of the secretariat.
Develop an inception report that includes a methodology, stakeholder mapping, work plan, risk matrix, and mitigation measures.
B. Rapid Assessment
Consultations with key stakeholders and conduct a rapid assessment of the current secretariat, mandates, workflows, staffing, and skills, and identify capacity gaps and strengthening needs
Review the functions of the secretariat, staffing, availability of job descriptions/ TOR, qualifications, competencies, experience, etc, in line with the act.
Share findings with the Ministry of NHSR&C and PNMC for their views and comments.
C. Revamping of the PNMC Secretariat
Propose a revised organisational design (organogram including units), with clear responsibilities, reporting lines, staffing norms, appointment criteria, competency profiles, and KPIs for an efficient and modernised secretariat.
Based on the assessment, restructure/reorganise the three units as follows:
Management Unit: Responsible for the overall management of the PNMC, including implementing Council and Ministry decisions, setting strategic direction and monitoring its implementation, handling litigation, overseeing human resource recruitment and deployment, and managing operational functions such as information systems, procurement, and finance.
Education Unit: Serves as the academic backbone of the PNMC, tasked with maintaining and enhancing the educational standards of nursing and midwifery programmes throughout Pakistan.
Quality Assurance Unit: Focuses on monitoring nursing and midwifery institutions, enrolled students, and the workforce, as well as driving continuous quality improvement initiatives in education and practice.
As per government policy, identify the work for which positions could be outsourced and mention the process in the report.
Draft and share the revamped report with stakeholders, incorporating their feedback.
D. Finalise the revamping report
Finalise the report and facilitate notifications of the three units.
Sustainability: Capacity Building, Institutionalisation, and/or Transition Planning
Focal Point Engagement: Focal points nominated by the Joint Secretary (Admin), Director General (Health), and Secretary PNMC will be actively involved in capacity development activities wherever feasible.
Capacity Building: The TA will integrate capacity building across all stages of the process. This will include the hands-on participation of nominated PNMC and Ministry staff in the review and design activities, structured orientation sessions on the new systems and processes, and the preparation of a comprehensive reference report for ongoing use.
Institutionalisation: The proposed reforms will be embedded within the PNMC Secretariat by aligning all new roles and processes with the PNMC Act and its mandated functions.
Transition Planning: The implementation plan will outline clear, sequential steps for phasing in the new structure and processes.

Background
- The Government of Khyber Pakhtunkhwa (KP), under its provincial reform roadmap, is advancing the objective of strengthening institutions and expanding access to quality healthcare for every citizen. Within this framework, public health emergency preparedness is a critical priority.
- Recent emergencies—including COVID-19, dengue, measles, and climate-related shocks—have revealed persistent weaknesses in district-level response, coordination, and institutional readiness.
- The Joint External Evaluation (JEE) and the National Action Plan for Health Security (NAPHS 2024–2028), which includes a provincial chapter for KP, recommend simulation exercises and after-action reviews as key tools for building preparedness and strengthening institutional capacity.
- The province is currently revising its health policies, presenting an opportunity to integrate structured simulation exercises into a sustainable provincial training system. This approach will institutionalise learning, improve district-level capacities, and ensure preparedness measures are embedded within the broader health reform and policy agenda.
- Embedding such exercises into routine training systems will not only align KP with IHR and national frameworks but also provide a replicable model of sustainable health security and resilience.
Goal and Objective(s)
The overall goal of this this technical assistance is to strengthen public health emergency preparedness and response capacity in vulnerable districts of KP by institutionalising simulation exercises and fostering systematic learning for resilient, evidence-based action.
We will achieve this by pursuing three objectives:
Objective 1: To adapt and institutionalise a standardised simulation exercise package for public health emergency preparedness, tailored to the needs of selected vulnerable districts.
Objective 2: To conduct simulation exercises that enhance district health teams' capacity for coordinated public health emergency response, while embedding these exercises into Sustainable Training System (STS) for sustainability.
Objective 3: To conduct After Action Reviews (AAR) to generate lessons learned and policy recommendations to inform the scale-up of simulation exercises across other districts in KP.
Deliverables
The consultant will work to achieve the following deliverables:
Adapted training materials and scenarios included in the package.
Monitoring tool developed and integrated into the training package.
Training-of-trainers completion report for at least two batches.
Cascade training pilot delivery report, including participation of PHC facility in-charges.
Recommendations for refinement and scale-up of the training package.

Background and Problem Statement
Background
Pakistan is a signatory to the International Health Regulations (IHR 2005) and under this obligation is committed to strengthening its health security core capacities within the health systems, with provincial and district leadership playing a critical role in preparedness and response.
New diseases, like COVID-19, are emerging at unprecedented rates disrupting people’s health and causing social and economic impacts. Millions of passengers travel on airplanes each year, increasing the opportunities for the rapid international spread of infectious agents and their vectors.
Dependence on chemicals has increased, as has awareness of the potential hazards for health and environment, like climate change and air pollution. As the globalization of food production increases, so does the risk of foodborne diseases
Recent Joint External Evaluation country report (JEE 2023) and National Action Plan for Health Security (NAPHS 2024–28) highlighted gaps in understanding and application of IHR nineteen core capacities and functions at the provincial and district levels.
Senior health managers (Directors at DG Health Services Office and district health authorities) remain under prepared to operationalise IHR core capacities within the health systems, particularly in prevention, detection, response to outbreaks, multisector coordination and collaboration.
Without structured trainings and provision of guidelines, provincial and district leadership risk a disorganised state of preparedness and a coordinated response to health emergencies and pandemics costing lives and risk to economy.
Senior health managers at provincial and district level in Punjab have a limited understanding of the needs of core technical capacities to operationalise IHR functions. This is aggravated by the frequent turnover of government officials. This leads to limited multisector coordination, weak preparedness, and fragmented responses to public health emergencies.
Goal and Objective(s)
The overall goal of this TA is to strengthen the capacity and understanding of senior health managers in Punjab to effectively plan, allocate resources and operationalise IHR core functions and contribute to provincial and national health security.
We will achieve this by pursuing 3 objectives:
- Objective 1 - Strengthen Leadership Capabilities: Deliver structured training to senior managers, focusing on the core capacities of the International Health Regulations (IHR) with one health approach.
- Objective 2 - Enhance Practical Skills: Provide managers with practical tools and knowledge to improve surveillance, risk communication, and multisectoral coordination efforts.

The TA will provide structured training for senior and mid-level health managers across Punjab on IHR (2005) core functions and their roles and responsibilities. The approach includes:
Phase 1 –Inception & Training Design: Needs assessment, development of training modules, and Inception Report & Slide Deck.
Phase 2 – Training Delivery : Conduct seven provincial-level training workshops for senior and seven for mid-level health managers from all districts of Punjab (full-day - 25 participants in each workshop), including plenary and group work.
Phase 3 –Documentation & Institutionalisation: Produce a final report and training toolkit for institutional use, including recommendations for routine integration into H&PD training systems.
Sustainability: Capacity Building, Institutionalisation, and/or Transition Planning
Training modules will be co-developed with H&PD, ensuring local ownership and adaptability.
Training materials will be institutionalised and sustained within the Directorate General of Health Services and PHDC for use in future capacity-building cycles through the PHDC/DHDCs network.
The TA will transition knowledge to nominated focal persons in H&PD to sustain momentum beyond E4H support.

Background and Problem Statement
Background
Pakistan is a signatory to the International Health Regulations (IHR 2005) and under this obligation is committed to strengthening its health security core capacities within the health systems, with provincial and district leadership playing a critical role in preparedness and response.
New diseases, like COVID-19, are emerging at unprecedented rates disrupting people’s health and causing social and economic impacts. Millions of passengers travel on airplanes each year, increasing the opportunities for the rapid international spread of infectious agents and their vectors.
Dependence on chemicals has increased, as has awareness of the potential hazards for health and environment, like climate change and air pollution. As the globalization of food production increases, so does the risk of foodborne diseases
Recent Joint External Evaluation country report (JEE 2023) and National Action Plan for Health Security (NAPHS 2024–28) highlighted gaps in understanding and application of IHR nineteen core capacities and functions at the provincial and district levels.
Senior health managers (Directors at DG Health Services Office and district health authorities) remain under prepared to operationalise IHR core capacities within the health systems, particularly in prevention, detection, response to outbreaks, multisector coordination and collaboration.
Without structured trainings and provision of guidelines, provincial and district leadership risk a disorganised state of preparedness and a coordinated response to health emergencies and pandemics costing lives and risk to economy.
Senior health managers at provincial and district level in Punjab have a limited understanding of the needs of core technical capacities to operationalise IHR functions. This is aggravated by the frequent turnover of government officials. This leads to limited multisector coordination, weak preparedness, and fragmented responses to public health emergencies.

E4H Punjab TA: Training of Trainers for Smog-Related Health Crisis Response
Evidence for Health (E4H) is a Foreign, Commonwealth & Development Office (FCDO)-funded programme aimed at strengthening Pakistan's healthcare system, thereby decreasing the burden of illness and saving lives. E4H (2023–2027) provides technical assistance (TA) to Punjab, Federal, and KP governments, implemented by Palladium in partnership with Oxford Policy Management (OPM).
Through its flexible, embedded, and demand-driven model, E4H supports governments to achieve a resilient health system that is prepared for emergencies, responsive to evidence, and delivers equitable, quality, and efficient healthcare services. Specifically, E4H delivers TA across three outputs:
Output 1: Strengthened integrated health security and preparedness.
Output 2: Strengthened evidence-based decision-making for accountability and performance.
Output 3: Improved implementation of Universal Health Coverage (UHC).
Background and Problem Statement
Background
- E4H supported the Health and Population Department in addressing the smog issue. A TA was initiated at the request of the H&PD to design a public awareness campaign addressing the health risks associated with smog. E4H played a central role in developing the campaign strategy, tailoring messaging and IEC materials for diverse population groups.
- Against this backdrop, the proposed Technical Assistance aims to support the Health & Population Department (H&PD) in strengthening preparedness for smog-related health crises by developing a cadre of district-level master trainers through structured Training of Trainers (ToT) workshops. This initiative seeks to strengthen the capacity of health workforce in Punjab and ensure continuity, efficiency, and quality in the management of seasonal respiratory health risks linked to smog.
- By focusing on workforce capacity-building and linking content to the LMS/PHDC/DHDCs, the intervention contributes to key health system building blocks: Health Workforce, Service Delivery, and Health Information Systems.
Punjab lacks a sustainable pool of certified trainers within P&SHD/DGHS to cascade smog-related health crisis response training. Reliance on ad-hoc workshops limits scale, quality, and continuity, leaving frontline health workers underprepared for annual smog surges.
Goal and Objective(s)
The overall goal of this TA is to establish a cadre of master trainers capable of institutionalising and cascading smog-related health crisis response training across Punjab.
We will achieve this by pursuing 3 objectives :
Objective 1: Develop an advanced Training of Trainers (ToT) curriculum integrating IDSR protocols and respiratory crisis management.
Objective 2: Train 50 master trainers selected from previously trained participants, equipping them with facilitation and simulation-based skills.
Objective 3: Standardise training materials, SOPs, and IEC content for provincial rollout, and strengthen DHAs’ capacity to independently deliver refresher and cascade trainings beyond the TA lifecycle.

This Technical Assistance will strengthen Punjab’s capacity to manage smog-related health crises by embedding ToT modules within the provincial training ecosystem. By equipping district-level master trainers with both technical expertise and facilitation skills, the intervention creates a sustainable cascade model for frontline health worker preparedness. Embedding the curriculum and materials within the Learning Management System (LMS), the Provincial Health Development Centre (PHDC), and District Health Development Centres (DHDCs) will ensure institutional ownership, reduce reliance on ad-hoc training, and promote continuity and quality in training delivery year after year.
Alignment with other E4H TAs/investments
The TA directly builds on previous E4H phases: Phase I (smog crisis response training for 200 professionals) and Phase II (embedded communications campaign for public awareness). It operationalises the Sustainable Training System Framework by linking smog-specific modules with the LMS, PHDC, and DHDCs. This complements broader E4H workstreams on climate-sensitive health security, PHC workforce strengthening, and the digitisation of training resources.
Alignment with other donors
The TA aligns with the Government of Punjab’s climate adaptation and health resilience agenda, particularly under the Punjab Green Development Programme and commitments to improve air quality. It is consistent with WHO’s IDSR framework for surveillance and case management, UNICEF’s focus on child health and air pollution, and World Bank investments in environmental and health system resilience. By institutionalising digital content and enabling provincial and district-level hubs to lead, this TA ensures complementarity with other donor-supported reforms while avoiding duplication.
Scope of Work and Methodology
This Technical Assistance will operationalise the ToT model by strengthening training content, equipping a selected pool of master trainers, and embedding tools and systems for sustainable cascade delivery across Punjab. The approach combines curriculum enhancement, adult learning methodologies, and hands-on clinical simulations to ensure trainers are both technically proficient and capable of facilitating high-quality trainings at the district level.
The TA will conduct the following activities:
Curriculum Enhancement: Upgrade Phase I training modules into ToT manuals (trainer guides, facilitation notes, simulation packs).
Trainer Selection: Develop criteria and shortlist 50 trainers in consultation with H&PD/DHAs.
- Public Health/IDSR protocols (AURI, ILI, ALRI, SARI).
- Clinical management (SARI, asthma, hospital-acquired acute disease).
- Adult learning methodologies, facilitation, and clinical simulation skills.
- Toolkit Development: Produce a “Trainer Toolkit” (decks, SOPs, pre/post-tests, IEC templates).
- M&E and Reporting: Assess trainer competencies, collect feedback, and produce a final report with recommendations for cascade rollout.
Sustainability: Capacity Building, Institutionalisation, and/or Transition Planning
- Moreover, this TA will institutionalise and link the ToT modules with the Learning Management System (LMS), the Provincial Health Development Centre (PHDC), and District Health Development Centres (DHDCs) in line with the Sustainable Training System Framework developed in the previous year of E4H. By embedding the content digitally and at institutional training hubs, this TA ensures long-term sustainability and accessibility for refresher trainings and cascade rollouts.
- This technical assistance aligns with the Government of Punjab’s broader health systems strengthening goals. It directly supports Output Indicator 3.2 of the E4H Programme: Strengthened health workforce as a means to PHC for Universal Health Coverage (UHC).
- Capacity will be built through ToT workshops, ensuring participants can independently conduct trainings.
- Training materials, SOPs, and IEC content will be embedded within PHDC LMS and Health Education Wing repositories.
- Cascade training plans will be handed over to DHAs, enabling district-level replication each smog season.
