Aga Khan University Hospital is recruiting a Clinical Nurse Coordinator, Home Health Services based in Pakistan.

The successful candidate will be responsible for:

1. Coordinating all processes for patient care and ensuring timely delivery of all core processes.
2. Managing scheduling and staffing of individual cohorts.
3. Participating in multidisciplinary rounds.

You will be responsible to:

• Impart patient/family education
• Communicate effectively with all health team members, patients and their families and ensure their satisfied response.

You should have:

• A Diploma in Midwifery from an accredited school of Nursing & Midwifery with at least one year of experience
• A valid license from Pakistan Nursing Council
• Competence to plan and deliver care to patients with a variety of complex care needs
• Good interpersonal skills, particularly communication skills
• A willingness to work in all shift duties.

• Participates in multidisciplinary rounds as applicable and plan patient care accordingly.
• Acts as clinical resource person for staff and patients and conduct teaching sessions as needed.
• Provide remote assistance to staff working in home setting.

You should have:

• A Baccalaureate Degree in Nursing (BScN) from an accredited school
• Minimum three years of nursing experience
• Valid license from Pakistan Nursing Council
• Certification in Cardiopulmonary Resuscitation (CPR), Safe Medication Administration, Aseptic Techniques, I/V Cannulation
• In-depth knowledge of health and safety measures
• Good communication and interpersonal skills
• Willingness to work in shift duties
