Key Responsibilities:- Evaluate and manage patient medication therapy to ensure safety and effectiveness.
- Provide expert consultation to physicians and healthcare teams on drug selection, dosages, interactions, and potential side effects.
- Develop and implement medication therapy management programs.
- Educate patients on the proper use of medications and potential side effects.
- Monitor patient progress and adjust medications as needed to improve outcomes.
- Ensure compliance with hospital policies, regulations, and best practices in pharmacotherapy.
- Conduct research and stay updated with the latest developments in pharmacology and clinical guidelines.
- Collaborate with healthcare professionals to develop treatment plans tailored to individual patient needs.
- Participate in clinical rounds and multidisciplinary team meetings.
- Train and mentor pharmacy staff, interns, and students.
- Education: Doctor of Pharmacy (PharmD)
- Strong knowledge of pharmacology, drug interactions, and disease management.
- Excellent communication and patient counseling skills.
- Ability to work in a fast-paced healthcare environment.
- Strong analytical and problem-solving skills.
- Attention to detail and commitment to patient safety.

- Enter all patient data into the electronic medical record
- Re-schedule patient appointments as required via telephone calls
and written notification to patients.
- Screen and route patient calls to other departments efficiently, ensuring accurate registration, appointment scheduling, and follow-up appointment scheduling in the database.
- Coordinate messages between patients and physicians. If consult calls are received, they must be properly handled and routed to the correct provider.
- Other duties as assigned.
Qualifications and Skills:
- Must display a 'customer-first mentality
- English proficiency required (Listening and Speaking)
- Self-starter and self-manager (Complete Training will be provided)
- Must be able to provide excellent patient care and handle difficult situations if necessary.
- Must be organized.
- Must know how to document properly.
- Must be punctual and have reliable transportation.
- Must be willing to work as a team player with fellow peers and providers.
